Abstract

Kanker serviks merupakan salah satu kanker yang paling sering ditemukan di Indonesia dan sampai saat ini masih belum dapat teratasi dengan baik. Penelitian ini memiliki tujuan untuk mengetahui gambaran histopatologi pada pasien dengan lesi prakanker serviks dan kanker serviks di Rumah Sakit Khusus Kanker Siloam MRCCC Semanggi pada tahun 2018-2020. Penelitian ini menggunakan desain penelitian retrospektif dengan sampel penelitian sebanyak 108 pasien yang sesuai dengan kriteria inklusi. Instrumen penelitian yang digunakan adalah arsip Laboratorium Patologi Anatomi Rumah Sakit Khusus Kanker Siloam MRCCC Semanggi pada tahun 2018-2020. Berdasarkan data, pasien yang terdiagnosis dengan lesi prakanker dan kanker serviks pada tahun 2018, 2019, dan 2020 terdapat sebanyak 23 pasien (21,3%), 44 pasien (40,7%), dan 41 pasien (38%). Pasien yang terdiagnosis dengan lesi prakanker serviks paling banyak ditemukan pada rentang usia 20-29 tahun (47,6%) dan pasien yang terdiagnosis dengan kanker serviks paling banyak pada rentang usia 40-49 tahun (31,8%). Pada pemeriksaan HPV DNA, didapatkan 23 pasien (21,3%) dengan hasil positif HPV tipe High-Risk dan 2 pasien (1,9%) dengan hasil kombinasi positif HPV tipe High-Risk dan tipe Low�Risk. Berdasarkan tipe HPV yang didapat, HPV tipe 16 paling banyak ditemukan (27,8%). Teknik pengambilan spesimen yang dilakukan adalah dengan biopsi serviks pada 72 pasien (66,7%) dan histerektomi pada 36 pasien (33,3%). Setelah dilakukan pemeriksaan histopatologi, pada pasien dengan lesi prakanker serviks, didapatkan 26 pasien (61,9%) dengan LSIL dan 16 pasien (38,1%) dengan HSIL. Pada pasien dengan kanker serviks, didapatkan 43 pasien (65,2%) dengan karsinoma sel skuamosa dan 23 pasien (34,8%) dengan adenokarsinoma. Faktor�faktor risiko memiliki peranan yang penting dalam terjadinya lesi prakanker serviks yang dapat berkembang menjadi kanker serviks, terutama akibat terpapar dan terinfeksi oleh HPV. Deteksi dini dan pemeriksaan histopatologi penting dilakukan sebagai tindakan pencegahan terhadap kemungkinan terjadinya lesi prakanker dan kanker serviks dan sebagai petunjuk agar pasien mendapatkan tatalaksana yang cepat dan tepat./ Cervical cancer is one of the most common cancer found in Indonesia and up to date, it is still hasn't been handled well. The purpose of this study is to understand the histopathological description of patient with precancerous lesion and cervical cancer at MRCCC Siloam Cancer Comprehensive Center Hospitals Semanggi in 2018-2020. Retrospective study design is used for this study with 108 patients suitable with the inclusion criteria. The archives of the anatomical pathology of MRCCC Siloam Cancer Comprehensive Center Hospitals Semanggi in 2018-2020 are used as the instrument for the study. According to the data, the number of patients diagnosed with precancerous lesion and cervical cancer in 2018, 2019, and 2020 respectively are 23 patients (21,3%), 44 patients (40,7%), and 41 patients (38%). Most of the patients diagnosed with precancerous lesion were found at the age of 20-29 years old (47,6%) and most of the patients diagnosed with cervical cancer were found at the age of 40-49 years old (31,8%). HPV DNA test shows that 21 patients (21,3%) with positive result of high-risk HPV and 2 patients with positive result of the combination of high-risk HPV and low-risk HPV. From the test results, HPV 16 was the most found (27,8%). The sampling techniques used were cervical biopsy in 72 patients (66,7%) and hysterectomy in 36 patients (33,3%). After the inspection of histopathology test, from the patients with precancerous lesion, 26 patients (61,9%) were found with LSIL and 16 patients (38,1%) were found with HSIL. From the patients with cervical cancer, 43 patients (65,2%) were found with squamous cell carcinoma and 23 patients (34,8%) were found with adenocarcinoma. Risk factors have an important role with the occurrence of precancerous lesion which can develop into cancer, especially due to the exposure and infection of HPV. Early detection and histopathology inspection are important as a preventive measure against the possibility of precancerous lesion and cervical cancer and as a direction for the patient to receive the prompt and precise treatment.
