Abstract

Pemeriksaan tambahan untuk mendiagnosis kanker payudara salah satunya adalah pemeriksaan imunohistokimia. Pemeriksaan imunohistokimia merupakan suatu metode pemeriksaan protein intraseluler dengan pemanfaatan antibodi monoklonal dan poliklonal dalam mendeteksi antigen yang terdapat dipermukaan jaringan. Pemeriksaan IHK pada kanker payudara dapat dilakukan untuk menentukan terapi dan prognosis yang ditentukan oleh penanda reseptor hormon (ER dan PR), ekspresi HER-2/Neu, dan penanda apoptosis dan proliferasi sel (Ki-67 dan p53). Penelitian ini bertujuan untuk mengetahui status hormonal pada pasien kanker payudara di RS Siloam Semanggi tahun 2018, berupa usia, jenis kelamin, diagnosis patologi, dan hasil pemeriksaan IHK (ER, PR, HER2, dan Ki-67). Penelitian ini merupakan studi deskriptif retrospektif dengan menggunakan data hasil laboratorium patologi anatomik pasien kanker payudara di RS Siloam Semanggi sebanyak 208 pasien yang memenuhi kriteria inklusi maupun eksklusi. Hasil penelitian ini: kelompok usia frekuensi terbanyak 50-59 tahun sebanyak 34.1%. Jenis kelamin frekuensi terbanyak pada wanita sebanyak 99.5%. Ca mammae NST grade II dan III sebanyak 38.0%. ER dan PR positif 51.0%, HER2 negatif 56.7%, Ki-67 proliferasi tinggi sebanyak 82.7%. Subtipe Luminal B dengan HER2 negatif sebanyak 32.2%. Pasien dengan Luminal B HER2 negatif banyak pada usia 50-59 tahun sebanyak 26 pasien. Pasien Luminal B HER2 negatif dengan Ca mammae NST grade II sebanyak 27 pasien. Kata kunci: Kanker payudara, patologi anatomik, imunohistokimia, subtipe kanker payudara/Immunohistochemistry examination (IHC) is one of additional test to diagnose breast cancer. IHC examination is a method to check intracellular protein using monoclonal and polyclonal antibody to detect antigen in tissue. IHC examination determined by hormone receptor markers (ER and PR), HER-2/Neu expression, and apoptotic and proliferation markers (Ki-67 and p53) that can be used to determine therapy and prognosis. This study aims to determine hormonal status of breast cancer patient at Siloam Semanggi Hospital in 2018, in the form of age, gender, pathology diagnose, and result of IHC (ER, PR, HER2, and Ki-67). This study is a retrospective descriptive study using pathological anatomy laboratory results of breast cancer in Siloam Semanggi Hospital as many as 208 patients in accordance with inclusion and exclusion criteria. This study obtained the results: age group with the highest frequency is 50-59 years with 34.1%. The highest frequency by gender is woman with 99.5%. Carcinoma mammae NST with grade II and III, was found in 38.0% patients. Hormonal receptor with ER and PR positive was found in 51.0% patients. HER2 expression negative was found in 56.7% patients. High proliferation Ki-67 was found in 82.7% patients. Luminal B with HER2 negative subtype was found in 32.2% patients. Patients in 50-59 years with Luminal B with HER2 negative subtype was found in 26 patients. Patients in carcinoma mammae NST with grade II with Luminal B with HER2 negative subtype was found in 27 patients.
