Abstract
Latar belakang: Asma bronkial merupakan penyakit inflamasi kronis saluran pernapasan yang dapat menyebabkan kegawatan airway breathing akibat bronkospasme, edema mukosa, dan penumpukan sekret sehingga menimbulkan risiko hipoksemia. Penanganan cepat di IGD diperlukan melalui pemberian oksigen, monitoring saturasi oksigen, dan terapi nebulizer. Tujuan: Mengimplementasikan pemberian oksigen, monitoring saturasi, dan nebulizer untuk mengatasi risiko hipoksemia pada pasien asma bronkial di IGD RSUD Budhi Asih. Metode: Studi kasus deskriptif pada dua pasien asma bronkial di IGD RSUD Budhi Asih melalui observasi, pemeriksaan fisik, pemantauan frekuensi napas, bunyi napas, dan saturasi oksigen menggunakan pulse oximetry selama intervensi keperawatan. Hasil: Perawatan kedua pasien dilakukan dalam rentang 30–60 menit. Pada satu am pertama, pemantauan dilakukan setiap 15 menit, kemudian setiap 30 menit sesuai perkembangan kondisi pasien. Setelah intervensi, pasien pertama mengalami penurunan sesak dengan frekuensi napas 22 kali permenit, saturasi oksigen 95%, namun masih terdengar wheezing. Pasien kedua tidak lagi sesak, frekuensi napas 19 kali permenit, saturasi oksigen 99%, dan tidak terdengar bunyi napas tambahan. Kesimpulan: Pemberian oksigen, monitoring saturasi, dan nebulizer efektif meningkatkan oksigenasi serta mengatasi kegawatan airway breathing pada pasien asma bronkial di IGD. Kata kunci : Asma bronkial, Hipoksemia, Monitoring saturasi, Nebulizer, Pemberian oksigen / Background: Bronchial asthma is a chronic inflammatory disease of the respiratory tract that can cause airway breathing emergencies due to bronchospasm, mucosal edema, and secretion accumulation, resulting in a risk of hypoxemia. Rapid management in the Emergency Department is required through oxygen administration, oxygen saturation monitoring, and nebulizer therapy. Objective: To implement oxygen administration, saturation monitoring, and nebulizer therapy to manage the risk of hypoxemia in bronchial asthma patients at the Emergency Department of RSUD Budhi Asih. Methods: A descriptive case study was conducted on two bronchial asthma patients in the Emergency Department of RSUD Budhi Asih through observation, physical examination, monitoring of respiratory rate, breath sounds, and oxygen saturation using pulse oximetry during nursing interventions. Results: The treatment of both patients was carried out within 30–60 minutes. During the first hour, monitoring was performed every 15 minutes, followed by every 30 minutes according to the patients’ condition development. After the intervention, the first patient experienced reduced shortness of breath with a respiratory rate of 22 breaths/minute, oxygen saturation of 95%, although wheezing was still heard. The second patient no longer experienced shortness of breath, with a respiratory rate of 19 breaths/minute, oxygen saturation of 99%, and no additional breath sounds detected. Conclusion: Oxygen administration, saturation monitoring, and nebulizer therapy were effective in improving oxygenation and managing airway breathing emergencies in bronchial asthma patients in the Emergency Department.
