Abstract

Latar belakang: Secara global, Gagal Ginjal Kronis (GGK) menjadi penyebab kematian ke-12 dengan angka kematian 5–10 juta jiwa per tahun. Di Indonesia, dari 638.178 responden terdapat sekitar 1.149 kasus GGK, sedangkan di DKI Jakarta ditemukan 55 kasus dari 24.981 responden. Gagal ginjal kronis merupakan penyakit tidak menular yang berkembang progresif akibat penurunan fungsi ginjal selama ≥3 bulan sehingga mengganggu metabolisme dan keseimbangan cairan-elektrolit hingga menyebabkan uremia. Salah satu komplikasi yang umum adalah edema ekstremitas bawah akibat peningkatan tekanan hidrostatik pembuluh darah. Latihan rentang gerak aktif pergelangan kaki dan elevasi kaki 30° efektif mengurangi edema. Tujuan: Mengevaluasi efektivitas latihan rentang gerak aktif pergelangan kaki dan elevasi kaki 30° terhadap penurunan edema ekstremitas bawah pada pasien GGK di RSUD Budhi Asih Jakarta. Metode: Penelitian menggunakan desain studi kasus deskriptif pada dua pasien GGK yang menjalani hemodialisa. Data diperoleh melalui observasi derajat edema dan respons fisiologis pasien terhadap intervensi. Hasil: Setelah intervensi selama 3 hari, kedua pasien mengalami penurunan edema dari derajat II menjadi I. Pada pasien satu, kedalaman edema menurun dari 4 mm menjadi 2 mm dan waktu kembali dari 15 detik menjadi 3 detik. Pada pasien dua, kedalaman edema menurun dari 4 mm menjadi 1 mm dan waktu kembali dari 10 detik menjadi 3 detik. Kesimpulan: Latihan rentang gerak aktif pergelangan kaki dan elevasi kaki 30° efektif menurunkan edema ekstremitas bawah pada pasien GGK. Perawat berperan penting dalam memotivasi pasien untuk melakukan latihan tersebut. In Indonesian, out of 638,178 respondents, there were approximately 1,149 cases of CKD, while in Jakarta, 55 cases were identified among 24,981 respondents. Chronic kidney disease is a non-communicable disease that progresses due to a decline in kidney function over ≥3 months, disrupting metabolism and fluid-electrolyte balance and leading to uremia. A common complication is lower extremity edema resulting from increased hydrostatic pressure in blood vessels. Active ankle range-of-motion exercises and 30° leg elevation are effective in reducing edema. Objective: To evaluate the effectiveness of active ankle range-of-motion exercises and 30° leg elevation in reducing lower extremity edema in CKD patients at Budhi Asih General Hospital, Jakarta. Methods: This study employed a descriptive case study design involving two CKD patients undergoing hemodialysis. Data were collected through observation of edema severity and patients’ physiological responses to the intervention. Results: After a 3-day intervention, both patients experienced a reduction in edema from grade II to grade I. In Patient 1, edema depth decreased from 4 mm to 2 mm and recovery time decreased from 15 seconds to 3 seconds. In Patient 2, edema depth decreased from 4 mm to 1 mm and recovery time decreased from 10 seconds to 3 seconds.
