Abstract

Human Immunodeficiency Virus (HIV) merupakan virus yang mengancam kualitas hidup manusia. HIV adalah retrovius yang menggunakan RNA sebagai genomnya sehingga virus ini dapat bereplikasi dengan sangat baik. HIV akan menyebabkan penyakit yang persisten sehingga pasien yang terkonfirmasi HIV harus mengkonsumsi obat seumur hidup. Sangat penting bagi pasien HIV untuk mendapatkan terapi dan melakukan pemantauan kadar VL RNA dan CD4. Umumnya pemeriksaan rutin dilakukan 3-6 bulan sekali. Kadar VL RNA menunjukan berapa banyak virus didalam darah sedangkan CD4 merepresentasikan imunitas pasien. Normalnya VL RNA dan CD4 akan menunjukan hasil yang berbanding terbalik. Semakin tinggi VL RNA dan rendah CD4 maka kondisi pasien akan semakin buruk, begitupun sebaliknya. Papua merupakan salah satu provinsi di Indonesia dengan angka HIV tertinggi dengan penyebaran yang lebih cepat dibanding dengan provinsi lain. Untuk menanggulangi permasalahan ini pemerintah telah menyediakan terapi gratis untuk rakyat Indonesia. Terapi ini bernama antiretroviral yang bisa didapatkan secara gratis dan wajib dijalani oleh semua pasien yang positif HIV. Tujuan penelitian ini untuk mengetahui kadar VL RNA dan CD4 pada pasien HIV yang diterapi ARV di RSUD Abepura. Penelitian ini dilakukan secara cross sectional menggunakan data rekam medik dengan jumlah sampel sebanyak 70. Pengolahan data menggunakan uji statistik secara univariat dan Microsoft excel. Hasil didapatkan dengan karakteristik pasien lebih banyak berjenis kelamin perempuan (52,9%), usia 25—49 tahun (68,6%), dan heteroseksual (81,4%). Secara keseluruhan terapi menunjukan keberhasilan dengan kenaikan pasien RNA undetected sebanyak 24% dan kenaikan pasien dengan CD4 normal sebanyak 76% selama tiga tahun pemantauan. Keberhasilan terapi ini bisa terjadi jika adanya kepatuhan minum obat dari pasien, kerja sama yang baik antara tenaga kesehatan dan pasien, serta dukungan dari lingkungan pasien. / Human Immunodeficiency Virus (HIV) is a virus that threatens the quality of human life. HIV is a retrovius that uses RNA as its genome so that this virus can replicate very well. HIV will cause persistent disease so patients who are confirmed HIV must take medication for life. It is very important for HIV patients to receive therapy and monitor VL RNA and CD4 levels. Routine testing is generally done every 3-6 months. VL RNA levels show how much virus is in the blood while CD4 represents the patient's immunity. Normally VL RNA and CD4 will show inversely proportional results. The higher the VL RNA and the lower the CD4, the worse the patient's condition and vice versa. Papua is one of the provinces in Indonesia with the highest HIV rate with a faster spread than other provinces. To overcome this problem, the government has provided free therapy for the Indonesian people. This therapy is called antiretroviral which can be obtained for free and must be rtaken by all HIV positive patients. The purpose of this study was to determine the levels of VL RNA and CD4 in HIV patients treated with ARVs at Abepura District Hospital. This research was conducted cross sectionally using medical record data with a total sample size of 70. Data processing used univariate statistic and Microsoft Excel. The results obtained showed that the patient characteristics were mostly female (52.9%), aged 25-49 years (68.6%), and heterosexual (81.4%). Overall therapy showed success with an increase in undetected RNA patients by 24% and an increase in patients with normal CD4 by 76% during three years of monitoring. The success of this therapy can occur if there is compliance with taking medication from patients, good cooperation between health workers and patients, and support from the patient's environment.
