Abstract
Heart failure is a physiologic state in which the heart cannot pump enough blood to meet the metabolic needs of the body. The incident of heart failure recorded in Dr. Cipto Mangunkusumo’s hospital is 839. The major symptom of heart failure is fatigue. Prevalance of fatigue in Cleveland Ohio Hospital is 43,1%. One intervention to reduce fatigue is Home Heart Walk (HHW). The aim of this research was to determine the effectiveness of HHW toward the degree of fatigue in heart failure’s patients. Method: This research used Pre Experiment One Group Pretest-Postest design, in Dr. Cipto Mangunkusumo & PGI Cikini hospital during Mey-July 2015, 50 respondents with heart failure functional class 2 & 3 patients, and purposive sampling. Result: The results showed majority of the respondents are elderly (44%), male (64%), with normal BMI (50%), and 76% of them in class 2 on functional classification of heart failure. Before HHW’s intervention, the majority of systolic & diastolic blood pressures were in the category of prehypertension respectively (36% and 40%), all respondents performed normal pulse rates (90%), tachypnea respiratory rates (80%) and experiencing fatigue on scale 5 (52%), with the average of walking distance was at 202,44 meters. After HHW’s intervention, the majority of systolic and diastolic blood pressure were in the normal category (48%), all respondents performed normal pulse rate (100%), respiratory rate decreased to normal category (88%), the degree of fatigue decreased to moderate fatigue on scale 3 (50%) and the average of walking distance was increased into 254,76 meters. The test results showed there was significant difference in the degree of fatigue before and after HHW‘s intervention (p=0,000), there is a relationship between respiratory rate and the degree of fatigue (p=0,043), there are significant effect of HHW to the fatigue degree (p=0,000); but there was no correlation between age, sex, BMI, classification of heart failure, blood pressure, pulse rate and walking distance with the degree of fatigue (p>0,05). Discussion: The conclusion is HHW can reduce the degree of fatigue in heart failure patients during six weeks; then it is recommended for nurses to teach HHW and motivate patient to have self-exercise.
