Abstract
Pandemi COVID-19 yang muncul pada bulan maret tahun 2020 menjadi permasalahan yang sangat besar dikarenakan penyakit yang disebabkan oleh Severe Acute Respiratory Syndrome Corona Virus 2 (SARS-CoV-2), menyebabkan morbiditas dan mortalitas yang besar. Morbiditas dan mortalitas yang disebabkan oleh COVID-19 tersebut dapat didukung oleh kadar gula darah yang tinggi (hiperglikemia) yang dapat menyebabkan peningkatan kemungkinan terjadinya suatu infeksi dan peningkatan inflamasi dan koagulasi yang dapat memperburuk keadaan pasien COVID-19. Tujuan dari penelitian ini adalah untuk melihat perbandingan luaran klinis pada pasien COVID-19 dengan kadar gula darah tinggi dan normal yang dilakukan di RS UKI pada periode November 2020-Januari 2021. Metode yang digunakan yaitu dengan cara kohort retrospektif. Pengambilan data dilakukan dengan mencatat hasil rekam medis pasien COVID-19. Dari hasil penelitian didapatkan pasien COVID-19 dengan hiperglikemia cenderung mengalami perburukan dengan jumlah sebanyak 31 pasien (59,6%), sedangkan pasien COVID-19 dengan normoglikemia mengalami perbaikan klinis dengan jumlah pasien sebanyak 106 pasien (73,6%). Selain itu, hasil OR didapatkan sebesar 4,1 (IK95% 2,1-8,0) yang menandakan pasien COVID-19 dengan hiperglikemia memiliki kemungkinan 4,1 kali lebih besar mengalami perburukan dibandingkan dengan pasien dengan normoglikemia. Pada penelitian ini didapatkan juga HR sebesar 2,7 (IK95% 1,54-4,72) yang mengindikasikan pasien COVID-19 dengan hiperglikemia 2,7 kali lebih cepat mengalami perburukan klinis./ The COVID-19 pandemic that emerged in March 2020 became a very big problem due to the disease caused by Severe Acute Respiratory Syndrome Corona Virus 2 (SARS-CoV-2), causing great morbidity and mortality. The morbidity and mortality caused by COVID-19 can be supported by high blood sugar levels (hyperglycemia) which can increase the likelihood of an infection and increase inflammation and coagulation which can worsen the condition of COVID-19 patients. The purpose of this study was to compare clinical outcomes in COVID-19 patients with high and normal blood sugar levels. This research was conducted at the UKI Hospital in the period November 2020-January 2021. The method used was a cohort retrospective. Data collection was carried out by recording the medical records of COVID-19 patients. From the results of the study, it was found that COVID-19 patients with hyperglycemia tended to get worse with a total of 31 patients (59.6%), while COVID-19 patients with normoglycemia experienced clinical improvement with a total of 106 patients (73.6%). In addition, the OR results were obtained at 4.1 (95% CI 2.1-8.0) which indicated that COVID-19 patients with hyperglycemia had a 4.1 times greater likelihood of experiencing worsening compared to patients with normoglycemia. In this study, HR was also found to be 2.7 (95% CI 1.54-4.72) which indicated that COVID-19 patients with hyperglycemia were 2.7 times more likely to experience clinical deterioration.
