Abstract
Tuberkulosis adalah penyakit infeksi menular yang masih menjadi masalah di dunia termasuk Indonesia. Menurut WHO (World Health Organization), sebanyak sepuluh juta orang di dunia menderita Tuberkulosis. Data Riskesdas tahun 2018 angka kejadian Tuberkulosis di Indonesia mencapai lebih dari satu juta kasus, total kerugian ekonomis negara mencapai 136,7 miliar per tahun. Salah satu faktor risiko terjadinya Tuberkulosis adalah penyakit Diabetes Melitus Tipe 2 terutama jika kendali glikemik yang dinilai dari HbA1c, Glukosa Darah Puasa, Glukosa Darah 2 Jam Post Prandial tidak optimal. Pengobatan Tuberkulosis paru menjadi lebih panjang pada kendali glikemik yang tidak optimal, hal ini mempengaruhi kualitas hidup dan prognosis pasien. Pada penelitian pengaruh kendali glikemik pada Diabetes Melitus tipe 2 terhadap lama pengobatan Tuberkulosis paru di RSUP Persahabatan tahun 2019-2021, didapatkan populasi tertinggi terjadi pada rentang umur 56-65 tahun yaitu sebanyak 33 orang (40,2%) dengan didominasi oleh pasien berjenis kelamin laki-laki yaitu sebanyak 53 orang (64,6%). Pemeriksaan untuk melakukan diagnosis Tuberkulosis paru adalah pemeriksaan TCM MTb/Rif yaitu sebanyak 39 orang (47,6%) didukung oleh pemeriksaan foto thorax yang menunjukkan gambaran khas Tuberkulosis paru pada 69 orang (84,1%). Lama pengobatan Tuberkulosis lebih banyak terjadi selama 9 bulan yaitu pada 57 orang (69,5%) dengan nilai HbA1c >7% sebanyak 65 orang (79,3%), Glukosa Darah Puasa >130 mg/dL sebanyak 59 orang (72%), dan Glukosa Darah 2 Jam Post Prandial >180 mg/dL berjumlah 66 orang (80,5%). Pasien yang mengalami pengobatan lebih dari 6 bulan berjumlah 68 orang didominasi pasien dengan kendali glikemik buruk yaitu sebanyak 59 orang. Kendali glikemik dengan lama pengobatan Tuberkulosis paru terbukti memiliki hubungan signifikan dengan nilai p 0,001 dan pasien dengan kendali glikemik yang buruk lebih 8,741 kali lebih berisiko mengalami pemanjangan lama pengobatan Tuberkulosis paru./ Tuberculosis is a dangerous infectious disease and it is still a global health problem including in Indonesia. According to WHO (World Health Organization), there are ten million of individuals in the world diagnosed with Tuberculosis, this disease is concluded as one of ten infectious diseases that causing high mortality. According to Riskesdas in 2018, more than one million individuals in Indonesia are infected by Tuberculosis, it costs 136.7 billion to treat Tuberculosis patients per year. One of risk factor of this disease is Type 2 Diabetes Mellitus especially when glycemic control from HbA1c, fasting blood plasma glucose, and post prandial plasma glucose are not maintained. Tuberculosis treatment can be longer in patients with Diabetes Mellitus, it can worse their prognosis and quality of life. This research in patient with Type 2 Diabetes Mellitus with Lung Tuberculosis in Persahabatan General Hospital from 2019-2021 have found that, there are 33 patients in the age of 56-65 (40,2%) and are dominated by male in 53 patients (64,6%). Most patients are diagnosed by Xpert MTB/RIF assay in 39 patients (47,6%) and interpretation of chest X-ray are dominated with typical findings of Tuberculosis lesions with 69 patients (84,1%). Duration of Lung Tuberculosis treatment mostly finish in 9 months in 57 patients with HbA1c level >7% in 65 patients (79,3%), fasting blood glucose >130 mg/dL in 59 patients (72%), and post prandial glucose >180 mg/dL in 66 patients (80,5%). Duration of Lung Tuberculosis treatment is longer in 68 patients, mostly with poor glycemic control in 68 patients. Correlation between glycemic control and duration of Lung Tuberculosis treatment is significant with p-value result 0,001. Patients with poor gycemic control are at risk 8,741 times experience longer duration of Lung Tuberculosis treatment rather patients with good glycemic control
