Abstract

Pandemi COVID-19 berakibat pada pasien dengan gejala berat yang terinfeksi SARS-CoV-2 membutuhkan perawatan di ICU. Berbagai terapi suportif diberikan agar dapat mempertahankan hidup pasien COVID-19. ICU sebagai tempat menginap mempunyai konsekuensi pasien terpapar berbagai faktor risiko yang berakibat pada infeksi sekunder, yaitu masuknya Candida sp. dalam darah yang disebut kandidemia. Normalnya, Candida sp. hidup secara komensal dalam tubuh manusia, namun oleh karena berbagai terapi dan penggunaan prosedur invasif, Candida sp. dapat bermigrasi masuk dalam darah dan menurunkan kualitas hidup pasien. Penelitian ini dilakukan untuk mengetahui faktor yang berperan pada kejadian kandidemia pada COVID-19. Penelusuran artikel dengan pencarian “candidemia and covid-19” pada PubMed® dan Wiley diperoleh 72 artikel. Kemudian dari artikel tersebut diperoleh delapan artikel yang sesuai dengan kriteria inklusi dan eksklusi. Hasil penelitian yang didapatkan adalah kejadian kandidemia pada COVID-19 lebih banyak pada usia tua dengan gejala klinis yang tidak spesifik. Komorbid yang menjadi dasar dari penyakit ini adalah diabetes melitus, penyakit kardiovaskular, penyakit ginjal, keganasan dan penyakit paru-paru. Faktor risiko yang berperan akibat penggunaan ventilator mekanik, kateter vena sentral, kortikosteroid dan antibiotik spektrum luas. Selain itu diperoleh metode diagnosis, lama perawatan di ICU, terapi dan mortalitas pasien. Sehingga melalui penelitian ini, meningkatkan pengetahuan tenaga kesehatan dan pasien tentang risiko infeksi kandidemia pada COVID-19./ The COVID-19 pandemic has resulted in severely symptomatic SARS-CoV-2 patients requiring ICU treatment. Various supportive therapies are administered to COVID-19 patients in order to help them survive. The ICU as a place to stay has the consequence to expose the patients to various risk factors that result in secondary infection, namely the entry of Candida sp. in the blood, which is known as candidemia. Normally, Candida sp. live commensally in the human body. However, due to various therapies and the use of invasive procedures, Candida sp. can migrate into the blood and reduce the patients quality of life. This study was conducted to identify the factors that play a role in the incidence of candidemia in COVID-19. An article search with keywords “candidemia and covid-19” on PubMed® and Wiley yielded 72 articles. Of these articles, eight that matched the inclusion and exclusion criteria were obtained. The results showed that the incidence of candidemia in COVID-19 is higher in the elderly with non-specific clinical symptoms. The comorbidities that form the basis of this disease are diabetes mellitus, cardiovascular disease, kidney disease, malignancy, and lung disease. Risk factors that play a role include the effect of mechanical ventilation, central venous catheters, corticosteroids, and broad-spectrum antibiotics. In addition, the method of diagnosis, length of stay in the ICU, therapy, and patient mortality was obtained. This research is expected to increase the knowledge of health workers and patients about the risk of candidemia infection in COVID-19.
