Abstract

ABSTRAK Nama : Marshal Pardede Program Studi : D-III Fisioterapi Judul : Penatalaksanaan Fisioterapi pada Osteoarthritis Genu Sinistra. Pembimbing : Dr. Maksimus Bisa, SKM., SSt.Ft., M.Fis. Latar Belakang: keluhan seorang pria Lansia saat berjalan terasa nyeri pada lutut dan hasil didiagnosa dokter dengan pemeriksaan penunjang sebagai penyakit Osteoarthritis (OA). Karya tulis ini bertujuan tujuan untuk mengetahui penatalaksanaan fisioterapi pada kondisi osteoarthritis genu sinistra dengan menggunakan modalitas terapi berupa ultrasound, Transcutaneus Electrical Nerve Stimulation (TENS). Metode: rancangan penelitian studi kasus (case study) menggunakan seorang data pasien OA Genu Sinistra mulai dari 13 Februari - 10 Maret 2023. Pengumpulan data pada penelitian dengan wawancara menggunakan instrument Numeric Pain Rating Scale (NPRS), Barthel Indeks Scale dan Western Ontario McMaster Universities Arthritis Index (WOMAC). Pengukuran dengan goniometer dan Manual Muscle Test (MMT). Terapi dengan modalitas TENS dua kali seminggu, intensitas 15 MHz selama 15 menit, dan continuous, ultrasound dua kali seminggu dengan intensitas 1,5 watt/cm2 selama 5 menit, dan continuous. Hasil: pada minggu ke tiga setelah menjalani terapi sebanyak 6 kali, hasil yang diperoleh menunjukkan bahwa penilaian nyeri menurun pada gerak isometric fleksi dan ekstensi lutut kiri (dari 3 ke 2; dari 4 ke 2) dan oedema pada lutut kiri berkurang dari 41 cm menjadi 39 cm, sedangkan nyeri tekan, nyeri gerak aktif dan pasif pada lutut kiri tidak ada penurunan dan tidak ada peningkatan aktivitas fungsional Kesimpulan : Ultrasound, Transcutaneus Electrical Nerve Stimulation (TENS) dapat mengurangi nyeri isometric fleksi dan ekstensi serta mengurangi oedema tapi tidak efektif dalam peningkatan aktivitas fungsional pada kondisi oesteoartritis genu sinistra. Background: Complaints of an elderly man when walking felt pain in the knee and the results were diagnosed by a doctor with supporting examinations as Osteoarthritis (OA). This paper aims to determine the physiotherapy management of osteoarthritis of the left genu by using a therapeutic modality in the form of ultrasound, Transcutaneous Electrical Nerve Stimulation (TENS). Methods: The case study research design used patient data from OA Genu Sinistra starting from 13 February - 10 March 2023. Data collection in the study was conducted by interview using the Numeric Pain Rating Scale (NPRS) instrument, the Barthel Index Scale and the Western Ontario McMaster Universities Arthritis Index (WOMAC). Measurement with goniometer and Manual Muscle Test (MMT). Therapy with TENS modality twice a week, intensity of 15 MHz for 15 minutes, and continuous, twice a week ultrasound with intensity of 1.5 watt/cm2 for 5 minutes, and continuous. Results: in the third week after undergoing therapy for 6 times, the results obtained showed that the pain assessment decreased in isometric motion of left knee flexion and extension (from 3 to 2; from 4 to 2) and edema in the left knee decreased from 41 cm to 39 cm , while there was no decrease in tenderness, active and passive motion pain in the left knee and no increase in functional activity. Conclusion: ultrasound, Transcutaneous Electrical Nerve Stimulation (TENS) can reduce flexion and extension isometric pain and reduce edema but is not effective in increasing functional activity in left genu osteoarthritis.
