Abstract

Latar belakang: Rinosinusitis kronik (RSK) membutuhkan tambahan pengobatan pasca operasi untuk memaksimalkan pengobatan. Irigasi hidung dilakukan pada penderita rinosinusitis kronis setelah bedah sinus endoskopi fungsional dengan berbagai larutan. Hasil penelitian masih dijumpai adanya kontroversi.Tujuan: melakukan telaah berbagai literatur secara sistematis menilai efek klinis irigasi hidung dengan normal saline pada penderita RSK setelah operasi sinus. Metode: Pencarian literatur membandingkan efek irigasi normal saline dengan larutan lain pada RSK setelah opersi sinus yang berasal dari database PubMed, Springerlink, Cochrane dan Google Scoolar. Data dikumpulkan adalah skor endoskopik dan skor kualitas hidup. Hasil: Teridentifikasi 291 artikel, 7 artikel memenuhi kriteria. Setelah dilakukan review hanya 4 yang dapat dimasukan dalam metaanalisis. Analisis gabungan Mean Difference skor endoskopik pada ke-empat penelitian adalah 0,27 (95% CI:-1,22-1,75), dengan Z=0,54, P=0,73 (>0,05) artinya NS secara klinis ataupun secara statistik tidak ditemukan perbedaan yang bermakna dalam menurunkan skor endoskopi dibandingkan larutan lain. Analisis gabungan Mean Differen skor kualitas hidup pada ke-empat penelitian adalah 0.82 (95% CI:-5.81, 7.45), dengan Z=0,24 dan P=0,81 (>0.05) artinya Normal Saline secara klinis ataupun secara statistik tidak ditemukan perbedaan yang bermakna dalam menurunkan skor kualitas hidup dibandingkan larutan lain. Simpulan: penggunaan irigasi hidung efektif dalam memperbaiki gejala dan tanda pada pasien rinosinusitis kronis setelah operasi sinus, dan penggunaan normal saline dibandingkan dengan larutan lain tidak ditemukan adanya perbedaan yang bermakna dalam memperbaiki skor endoskopik ataupun skor kualitas hidup. Nasal irrigation is performed in patients with chronic rhinosinusitis after functional endoscopic sinus surgery with various solutions. The results of the study are still controversial. Objective: To conduct a systematic review of the literature to assess clinical nasal irrigation surgery with normal saline in patients with CRS after sinus. Methods: A literature search compared the effects of normal saline irrigation with other solutions on CRS after sinus surgery from the PubMed, Springerlink, Cochrane and Google Scoolar databases. The data collected were endoscopic scores and quality of life scores. Results: Identified 291 articles, 7 articles met the criteria. After review, only 4 were included in the meta-analysis. Combined analysis Mean Difference endoscopic scores in the four studies were 0.27 (95% CI: -1.22-1.75), with Z=0.54, P=0.73 (>0.05) meaning clinical NS or statistically no significant difference was found in lowering endoscopy scores compared to other solutions.
