Abstract

Infeksi oportunistik tertinggi pada orang HIV/AIDS adalah tuberkulosis. Tuberkulosis dan HIV merupakan dua penyakit kombinasi yang meningkatkan angka mortalitas di dunia. Infeksi TB pada pasien HIV terjadi karena menurunnya sel imun di dalam tubuh yang memudahkan respon imunologi terhadap Mycobacterium tuberculosis. Di Indonesia sendiri, kasus HIV/AIDS sebesar 478.784 kasus dengan 338.760 kasus HIV dan 140.024 kasus AIDS. Indonesia menempati posisi ke-3 di dunia dengan kasus TB sebanyak 824 ribu dengan kematian 93 ribu/tahun atau sekitar 11 kematian/jam. Pada tahun 2021, akibat penyakit TB sebanyak 1,6 juta orang meninggal diantaranya 187 ribu orang TB terkait HIV. Tujuan penelitian ini adalah untuk mengetahui hubungan antara faktor risiko dengan kejadian komplikasi TB pada pasien HIV/AIDS di RSUD Abepura. Studi ini dirancang secara cross sectional dengan menggunakan data rekam medik pasien dengan sampel berjumlah 100. SPSS digunakan dalam mengolah data pada penelitian ini dengan teknik univariat, bivariat dan multivariat. Hasil yang didapatkan menunjukkan karakteristik dari pasien HIV/AIDS yang mengalami komplikasi TB adalah laki – laki (56,8%; p-value= 0,302), usia >35 tahun (61,4%; p-value= 0,338), bekerja (61,4%; p-value= 0,207), pernah menjalani pendidikan ≥ 9 tahun (93,2%; p-value= 0,988), heteroseksual (90,0%; p-value= 0,235), nilai kadar CD4 <200 sel/mm3 (59,1%; p-value= 0,987) dan stadium HIV III-IV (97,7%; p-value= 0,002). Hasil dari analisis multivariat menunjukkan variabel stadium HIV (p-value= 0,012; OR 14,333) menjadi faktor paling dominan pada komplikasi TB pada pasien HIV/AIDS. Faktor jenis kelamin, usia, pekerjaan, pendidikan, orientasi seksual, dan nilai kadar CD4 tidak bermakna secara signifikan terhadap komplikasi tuberkulosis pada pasien HIV/AIDS di RSUD Abepura. Selain itu, ditemukan adanya hubungan signifikan antara nilai kadar CD4 dengan stadium HIV (p-value= 0,000). Tuberculosis and HIV are two combined diseases that increase mortality rates in the world. TB infection in HIV patients occurs due to decreased immune cells in the body which facilitates the immunological response to Mycobacterium tuberculosis. In Indonesia alone, HIV/AIDS cases amounted to 478,784 cases with 338,760 HIV cases and 140,024 AIDS cases. Indonesia occupies the 3rd position in the world with 824 thousand TB cases with 93 thousand deaths / year or around 11 deaths / hour. In 2021, as many as 1.6 million people died due to TB disease, of which 187 thousand people had HIV-related TB. The purpose of this study was to determine the relationship between risk factors and the incidence of TB complications in HIV/AIDS patients at Abepura Hospital. This study was designed cross-sectionally using medical record data of patients with a sample of 100. SPSS was used in processing data in this study with univariate, bivariate and multivariate techniques. The results obtained showed the characteristics of HIV / AIDS patients who experienced TB complications were men (56.8%; p-value = 0.302), age >35 years (61.4%; p-value= 0.338), working (61.4%; p-value = 0.207), has undergone education ≥ 9 years (93.2%; p-value= 0.988), heterosexual (90.0%; p-value = 0.235), CD4 value <200 cells/mm3 (59.1%; p-value = 0.987) and stage HIV III-IV (97.7%; p-value = 0.002). Results from multivariate analysis showed HIV staging variables (p-value = 0.012; OR 14,333) became the most dominant factor in TB complications in HIV/AIDS patients. Factors of sex, age, occupation, education, sexual orientation, and CD4 levels were not significantly significant for tuberculosis complications in HIV/AIDS patients at Abepura Regional Hospital. In addition, there was a significant relationship between CD4 levels and HIV stage (p-value = 0.000). Keywords: HIV/AIDS, complications, Tuberculosis.
