Abstract

Latar Belakang: seseorang yang mengalami tuberculosis baik sebelum pengobatan atau dalam masa pengobatan maupun setelah selesai pengobatan dapat terjadinya komplikasi, seperti pneumothorax. Tujuan: untuk mengetahui penatalaksanaan fisioterapi, problematik fisioterapi, patofisiologi dari problematik utama, dan penatalaksanaan modalitas fisioterapi pada pada pasien dengan kondisi water seal drainage et causa pneumothorax spontan sekunder. Metode: Metode penelitian studi kasus berisi tentang hasil intervensi fisioterapi menggunakan modalitas berupa breathing exercise, active exercise, dan massage therapy. Hasil: setelah dilakukan terapi sebanyak 3 kali didapat hasil penilaian sesak nafas pada T1: 3 menjadi T3: 3, aktivitas fungsional pada T1: 50 menjadi T3: 60, ekspansi thoraks pada T1: upper 0,5 cm, middle 0 cm, dan lower 1 cm menjadi T3: upper 2 cm, middle 3 cm, dan lower 2 cm, gerakan pernapasan pada T1: dominan abdomen menjadi T3: dominan abdomen, dan oedema pada T1: dextra 28 cm dan sinistra 29 cm menjadi T3: dextra 20 cm dan sinistra 19,5 cm. Kesimpulan: pemeriksaan fisioterapi yang dilakukan pada kondisi water seal drainage et causa pneumothorax spontan sekunder berupa asesmen fisioterapi, problematik yang didapatkan yaitu adanya keterbatasan ekspansi thoraks, sesak napas, spasme pada m. upper trapezius dan m. pectoralis mayor, nyeri gerak fleksi shoulder sinistra, oedema pa-da area pergelangan kaki, penurunan tonus otot tungkai bawah, dan keterbatasan aktivitas fungsional, pneumothorax yang terjadi pada pasien Ny DK merupakan jenis pneumothorax spontan sekunder, dan digunakan modalitas breathing exer-cise, active exercise serta massage therapy. / Background: someone who has tuberculosis either before treatment or during treatment or after completion of treatment complications can occur, such as pneumothorax. Purpose: to determine the management of physiotherapy, physio-therapy problems, pathophysiology of the main problems, and management of physiotherapy modalities in patients with water seal drainage conditions et causa secondary spontaneous pneumothorax. Methods: The case study research method contains the results of physiotherapy interventions using modalities in the form of breathing exercise , active exercise , and massage therapy. Results: after 3 times of therapy, the results of the shortness of breath assessment were obtained T1: 3 becomes T3: 3, functional activity at T1: 50 becomes T3: 60, thoracic expansion at T1: upper 0.5 cm, middle 0 cm, and lower 1 cm becomes T3: upper 2 cm, middle 3 cm, and lower 2 cm, respiratory movements at T1: dominant abdomen becomes T3: abdominal dominant, and edema at T1: dextra 28 cm and left 29 cm becomes T3: dextra 20 cm and left 19.5 cm. Conclusion: physiotherapy examination was carried out in conditions of water seal drainage et causa secondary spontaneous pneumothorax in the form of a physiotherapy assessment, the problems found were limited thoracic expansion, shortness of breath, spasms in m. upper trapezius and m. pectoralis major , left shoulder flexion motion pain , edema in the ankle ar-ea, decreased lower leg muscle tone, and limited functional activity. The pneumo-thorax that occurred in the patient Ny DK is a type of secondary spontaneous pneumothorax, and the modalities of breathing exercise, active exercise and mas-sage therapy are used.
